Independently ensure that the care plan is being followed, medication adherence and daily functioning is monitored, and review mental health and well-being indicators.

Coordinated Support for Complex Needs: medical, mental health, and legal systems
In-person and remote support as appropriate. Attend care planning meetings or calls as your "eyes and ears", ensuring client and family priorities for care are communicated effectively and understood by the care team.
Assist clients and families in navigating situations where past or current legal issues, substance abuse, or mental health concerns intersect with healthcare needs. Assist with communication around patient/resident rights in sensitive situations (e.g.; freedom from chemical restraint, privacy, or dignity when past or current legal or mental health issues may be involved).
All services focus on observation, communication, coordination, and rights protection — never medical diagnosis, treatment, therapy, or legal representation.
Family Peace-of-mind Services: Experience tailored to dual diagnoses and recovery histories, veterans and their families, and complex social and family concerns.
Facilitate independent living for as long as possible, connecting to external resources as appropriate.
Provide support during care transitions (whether toward greater independence or adjusting to a new facility setting).
Targeted support for clients experiencing medical discrimination due to their legal or mental health issues.
